[X-Newbies] HELP! I can not add a new account/user

I am using a Quicksilver Mac, System 10.3.8. I am the Administrator of 
this Mac, and the only account on this computer at present. I want to 
add an account (User), which I have done in the past with no problem. 
When I click on the + sign (which is not grayed out) nothing happens!! 
Have I forgotten something, am I doing something wrong?
