[X-Newbies] HELP! I can not add a new account/user

I have tried everything I know, I have restarted, I have signed out and 
shut off the power. Preferences are unlocked and still when I click the 
+ button nothing happens.

I am trying to set up an account for my assistant, which in the past I 
have never had a problem doing but now it seems as as if the + button 
on the Preferences is dead (even though it is not "grayed out"!  Am I 
doing something stupid, am I losing it are do I I have a real problem 
with the system??
