Should be your admin username (the main/first account you setup in OS X),
eg: rnagle  and that user's password.

Failing that, edit /etc/cups/cupsd.conf as follows:

#Encryption Required
<Limit GET>
#AuthType Basic
AuthType ShadowHash
AuthClass System

ie: comment out #AuthTypeBasic and add the next line.
