I have the form. It says US Porto Rico and St. Thomas. I've always 
wanted to live in St. Thomas. Maybe you can find a friend, like an 
Office Max employee to let you use their street address for the rebate 
submission then have them forward the check to you in the mail. You 
could enter the store with a self-stamped addressed envelope. Better 
phone first to have them hold one for you in the Cage.
