Cendyne rebate for DVD burner

Does any one have the rebate form from cendyne for the DVD burner? I 
would like a copy, please.
I may be able to buy one locally and can not find the rebate form and 
do not want to buy on the hunch that it is in the box.
PS Office Max (2 of them locally) has not had this drive in stock. I 
tried ordering it on their 800 number but its back ordered.
Staples has the same drive at $299. Just need to check their price 
matching policy :-)
