I don't have answers to all of your questions, but I can answer some. 

The lastest version of iListen does run on Panter (OS 10.3).

You will need help from your sighted freind, but all iListen commands are 
AppleScript, and are available for editing. Although there is no built in 
functionality to confirm commands by voice, the scripts could be edited 
to provide vocal feedback for the commands you use.

Another posibility is the the speach recognition built into Panther. This 
doesn't allow for dictation, but it does allow switching between 
applications by voice, or other commands.
