It's all about distribution rights. Banks and companies divvy up areas, states, and whatnot. The service is new. It's kind of amazing that it's available all over the US, even. American Top 40 radio used to test market in the San Fernando Valley, and if things flew there, they went into syndication elsewhere. A US-wide roll-out of the service shows that Apple is serious, and you can rest assured they aren't going to refuse business from elsewhere, unless they're constrained, by contract, terms of service with credit companies, etc. ~flipper
