It has to be called 'favicon.ico'. It has to be in the root of the website
(like http://www.site.com/favicon.ico). It is a Windows bitmap file
exactly 16x16 or 32x32.

I use the Favicon creator
(http://www.chami.com/html-kit/services/favicon/) to create mine. It
convert from many file formats.
