Try removing it from the Office startup items: /Applications/Microsoft Office X/Office/Startup/Word/PDFmaker.dot Move that file somewhere else (do the same for the PDFmaker files in the Excel and Powerpoint folders), and I think you'll be rid of that toolbar.
