There is one piece of spam which consistently gets through to my inbox. I always mark it junk but it keeps getting through. I thought Mail was supposed to learn. It's an ad for drugs, including viagra, it comes with an attached gif with a list of drug prices. The subject line is always nonsense and the to and from fields always different. But i can spot it a mile away.
