I just signed up with syracuse.net. They have dial-up numbers all over the US and a few places in Europe and Japan. I've been with them less than a month, but so far I am very pleased with their service. I had to call technical support once when I set up my e-mail and they were patient and efficient. They have a special rate of $10 for the first month to let people try them out. You can then get an introductory rate of $55 for six months. After that rates depend on how long you sign up for.
