[iBook] Lost menu item

I have lost my input menu item (No American flag). In the International 
system preferences I have Character Palette, Keyboard Viewer, 
TypeIt4Me, and U.S. checked. The U.S. is grayed out. I also have "Show 
input menu in menu bar" checked, but if I leave the preference pane and 
come back, it is unchecked.

Any ideas to get the menu item to display again?
