[iBook] Printing to custom page sizes?

This isn't iBook-specific, but probably of general use. Specs are:  
G3/900 iBook, 640MB RAM, OSX10.4.6.

I'm trying to print something on 3x5 index cards -- specifically, the  
planner templates at http://www.diyplanner.com/templates/official/ 
hpda -- and have run into nothing but frustration.

I created a custom 3x5 paper size, and made sure the file was set (in  
Page Setup) to use it. And it insists on thinking I'm printing to  
letter-size paper, centers the 3x5 card on the paper, and spits out  
either nothing or the first two inches of the printout on the last  
two inches of the card.

I've tried printing to a Lexmark Optra E312L (PostScript) laser  
printer and a Canon S820 inkjet, both before and after downloading &  
installing updated drivers from the vendors, with identical results.  
Apple's Help didn't turn up anything useful. A possible clue: I can  
select the 3x5 paper size when Page Setup is set for "any printer,"  
but when I select either printer all my custom sizes are greyed out.  
Perhaps I need to enable something, but what? I don't see anything in  
System Prefs that would work.
