[iTunes] What happens during Email Accounts syncing between iPhone and iMac?

I suspected this. It seems totally useless to continually sync email  
accounts after the first time. I can't see how it accomplishes anything.

I was under the impression that I had. What do you suggest that is  
different from my current settings?
