[G4] Radeon Keeps Me From Booting

I, too, wonder why you say only three PCI slots are supported. That makes me 
think that you may have other problems that just haven't shown up yet.

As to the booting problem...
If it worked for three months and then went south, it appears that it reached 
its MTBF point (Mean Time Before Failure) and died on you.

Otherwise, I would suggest:

1. Remove ATTO SCSI card.
2. Move Radeon card to different PCI socket.
3. Remove hub from connection.
4. Change connection socket of hub.
5. Re-arrange all PCI cards.
6. Try Radeon card in different Mac.
