[G4] Wake from Sleep Problems with USB 2.0 PCI Card

You already are at 4.2.8 - usually when you first installed MAC OSX it 
should have told you if your firmware was dated and told you how to 
reload it from the boot disk (and really, you could just update it by 
booting from an OS 9 CD - not re-installing).
