I was assuming a brand new volume/partition via Erase and Install for 
the OS 9 Driver check box to work.  That was the way I've done it.

Sorry, it turns out that the OS 9 would be on the "Restore" set of CDs 
that came with the machine.  This may help from a Google search for "OS 
9 on OS X Install disks":

Hopefully, any Restore set for the MDDs of your vintage would work.
