A similar thing happened to my sister, but in reverse. Prior to visiting the Getty Museum (where you're not supposed to use flash), I set up the camera to not use flash. She took a picture and the flash went off, with the expected repercussions from the guard. She had accidentally touched the button that cycled between various options. I prefer the manual control of my Nikon 995. If I want to use flash, I pop up the flash unit. Otherwise I don't have to worry about some tiny button with an icon I can't remember is on or off.
