Before you can create a folder, you have to select where you want the 
folder to be created. Often there is nothing selected in a finder 
window. Click on a folder on inside an open folder.

A similar problem occurs in pre-X choosers where people see a dark line 
but don't realize they still have to click on it to highlight what they 
want to select.
