Did you use Addressbook to set-up your autofill profile? This is where
Safari gets its info from.

You have to fill out a vCard in Addressbook with your pertinent info first.
Then specify this vCard as "my card".

Then when using Safari you have to sometimes click in the first field to be
filled in before selecting autofill. Some sites have too many blanks and
seem to confuse Safari.
